Are you a family member looking to learn more about Evoke Health and our communication portal? This section is for you.

We charged a monthly subscription fee to long-term care homes based on features and size of the home. Evoke Health's family and resident portal is free to residents, their families, and staff for the long-term care homes that have subscribed to our service.

Long-Term Care homes under a contractual agreement with Evoke Health to use the Evoke Health portal will electronically receive access to residents, their families (Power of Attorney and/or Substitute Decision Makers), and staff

Information within the Evoke Health portal is pulled directly from the Electronic Health Record System (such as PointClickCare) and can be supplemented with self-uploaded information.

Evoke Health employs encryption protocols equivalent to those used in online banking. We prioritize the privacy and security of each long-term care resident, ensuring compliance with HIPAA, PHIPA, and PIPEDA standards.

Click the “forgot password?” link on the Evoke Health sign-in page to reset your password.

In Evoke Health, you will see your name at the top right. Click on it, and you will see Settings. From there, you can adjust what notifications you receive from the community. If you plan on stopping all notifications, you should make sure to log in regularly to avoid missing any important updates.

Due to privacy and security concerns, you will need to go into the Evoke Health portal to see messages. Email isn't a secure place to share health updates and is a violation of HIPAA and PHIPA.
